How to treat SAM patients with CHD or renal diseases?

This question came in to the en-net postbag from Ahmed Salih: I am pediatrician & worked responsible on nutritional rehabilitation ward & l 'd like to ask whether those patients with C.P,CHD,renal diseases with SAM can be included in the programme of SAM ,WHO Modules & get benefit? with best regards. We have had some previous discussion on the topic, related to hepatitis http://www.en-net.org/question/119.aspx but it may be that there is further guidance out there for other conditions? Many thanks
