5 Years project in one district. What is the time to say "No Intervention"?

Dear All,
What is the time to graduate from X district and not to run CMAM program any more because you reached to all affected children?. What is the method used if you don't have baseline data per one Health facility? and you have it only for the entire HFs district while your intervention is only in some HFs?
What will happen if we have total children under five years by name, and we are monitoring our achievements by children names, do you think will not have double counting?, do you think we will have accurate coverage and to say "it is time for graduation of this X district from Malnutrition"?
