Selection of CMAM implementing facilities in a development context

Does anyone know of any documents that gives detailed guidelines for selecting CMAM implementing health facilities at the district level?That has details on how to factor in coverage, equipments and supplies at the facilities, population covered etc. into selection. Thanks.
